Level 1 is designed to give you gentle, practical guidance so you can take the lead with your supports.
Support Coordination (Level 2)
We provide tailored, person-centred support to help you:
- Understand your NDIS plan
- Explore your goals and priorities
- Connect with the right service providers
- Coordinate and schedule your supports
- Build the skills and confidence to manage your own plan
- Navigate changes or challenges as they arise
- Prepare for plan reviews with clarity
Our approach is calm, clear, and responsive — suitable for participants of all ages, backgrounds, and support needs.
Specialist Support Coordination (Level 3)
For participants with more complex needs, we offer:
- High-level coordination and problem-solving
- Crisis planning, response, and stabilisation
- Support for psychosocial, behavioural, medical, or housing complexity
- Collaboration with healthcare teams, therapists, and multiple providers
- Assistance during periods of change or increased vulnerability
- Strong advocacy to ensure safe, reliable, and effective supports
This service draws on extensive nursing and disability experience to support participants with multifaceted or high-risk needs.
Capacity Building & Ongoing Support
We help you strengthen your independence by supporting:
- Daily living and life skills
- Communication, decision-making, and self-advocacy
- Understanding your rights and options
- Building community connections
- Increasing confidence in managing appointments, services, and supports
